Support Coordination
We have experienced Support Coordinators who have worked in the field for over three years and know NDIS systems and services.
Support Coordinators will work closely with participants and their families to understand their plan, identify, and assist them to choose providers and services. This can assist in offering participants and their carers choice and control to achieve their goals, connect with services of their preference and participate in meaningful activities of their choice.
We are big on providing participants voice and choice with their NDIS plan and up to date communication to enable them to navigate complex NDIS and services systems.
Our support coordinators can assist you to:
- Identify the services and service providers that best meet participants’ needs and allow them to achieve value for money and exercise their choice and control
- Connect participants with a dedicated support team and coordinate service providers and supports (including mainstream, informal, community and funded supports); or build their capacity to coordinate participants’ own supports
- Get quotes for services and set up Service Agreements with providers
- Source suitable accommodation and respite options
- Link participants into a job, or other community activities
- Coordinate assessments, reports, and health and medical specialists
- Respond to a crisis; or identify solutions for managing risky situations, such as ageing carers
- Plan and explore options for times of key life transitions, such as leaving school or moving home
- Help participants explore advocacy services if this is something they require
- Monitor their progress towards their NDIS goals and the performance of the support services they use
- Request a plan review if the participant plan does not meet their needs, and review their goals prior to their NDIS plan review.
