Community Nursing
Professional nursing care and services are provided in the participant’s homes by qualified nurses.
Community Nurses work with participants and their families to prevent disease, maintain health and treat any existing health problems.
Community nurses aim to support and maintain participants' independence, safety, and healthy lifestyle, at the same time as assisting participants’ carers.
These services can include support with:
- Personal hygiene management
- Basic medical care and medication
- Complex wound management
- Source suitable accommodation and respite options
- Subcutaneous injections
- Complex bowel management care
- Enteral feeding and management (nasogastric tube-jejunum or duodenum)
- Tracheostomy management
- Bladder catheter management (indwelling catheter, internal/external catheter, suprapubic catheter)
